For travel beyond the 50-mile radius of 45230, additional fees are assessed as follows and applied to your total balance:

• Beyond 50 miles, within 120 miles: $200
• Beyond 120 miles, within 300 miles: $400
• Beyond 300 miles, within 500 miles: $600
• Beyond 500 miles, within the continental United States: $1000
